Hawthorns Care Centre is situated in Peterlee and provides care and rehabilitation services for those aged 18 and above requiring Specialist and Complex Neurological and Respiratory Care.

The Hawthorns provides neurological care services for those recovering from traumatic or non-traumatic brain or spinal injury.  This includes support and enablement for those with long term neurological conditions and a higher dependency floor where care is provided for people with complex neurological and respiratory care and rehabilitation needs.

We provide active rehabilitation packages as well as long term and respite placements. Our goal orientated therapy programmes are tailored to an individual’s ability and potential.

Our experienced and highly qualified care and therapy teams, led by a Consultant in Rehabilitation Medicine, deliver person-centred care to ensure the individual remains in control of their lives. Our purpose-built accommodation has been specifically created for adults with complex neurological, physical or cognitive needs.

Our specialist care service for individuals who require higher dependency nursing and multidisciplinary team intervention, includes patients with invasive and non-invasive ventilation needs those with complex respiratory needs or who are minimally conscious. We can provide SMART (specific, measurable, accountable, realistic and time limited) assessment and treatment and are proud to have Headway accreditation as an approved provider.

Client groups

The neuro-rehabilitation team works with people aged 18 and above with acquired and long-term neurological conditions, helping them to achieve maximum independence in all aspects of daily life. 

We have been operational for the last 10 years and has looked after clients with numerous neurological conditions such as subarachnoid haemorrhages, hypoxic brain injuries, spinal injury, ventilation for clients both long term and short term, respite, behavioural problems, aggression, multiple sclerosis, Guillian Barrie, cerebral palsy and head injuries. This has equipped us with a wealth of experience, knowledge and understanding.

We  are the main provider for ventilation services for patients who need longer term ventilation in the community. Often these patients are unable to go home due to complex medical reasons. For some patients it may be that they need to step down from the acute sector for a period of time before going back to the spinal injuries unit for weaning. For others it may be a long scale goal to be back in the community home. All our ventilated clients are screened by the home ventilation team as being suitable for community care before being assessed by our team.

We take patients with complex neurological conditions who are unfortunately unable to be discharged straight back home and those who will require a period of rehabilitation before transfer home or to supported living. We admit patients who are minimally conscious; those who have tracheotomies; ventilated patients and those who have complex behavioural or cognitive problems.

We are not a behavioural treatment unit, although some altered behaviour can be associated with an acquired brain injury. If clients have complex behavioural needs they would be best served by a behavioural unit such as those at our independent hospitals.

We promote and support family involvement and encourage them to offer positive feedback to the patient. The nature of many client’s injuries mean that they can forget what they were like when they arrived and what they have achieved, so this support can be invaluable. We can use visual reinforcement in the form of pictures or video to help the person see how far they have come along the rehabilitation path. Family often plays a large part in the person’s rehabilitation. We encourage them to give a comprehensive history of the person, including their likes and dislikes. This is particularly helpful initially whilst we get to know the person and learn all about them. Knowing what they were like before their injury as well as after can also be very helpful in cognitive therapy and behavioural work, helping to gain a better understanding of their likes, dislikes, needs and personality. We also provide regular interim reports and reviews involving commissioners, clients, families, nurses and members of the multidisciplinary team.

Our approach to care

Our approach to care is to provide a safe, homely environment in which our clients can achieve their maximum potential. We facilitate this through open and honest communications with the client, their families and all healthcare professionals involved. Regular MDT meetings in which goals are set and reviewed enable us to ensure we are meeting the individual needs of our clients, using a holistic approach to their rehabilitation journey. At the heart of the Hawthorns team is a shared passion to provide the best care possible for each individual: this is achieved through good leadership, excellent communication, a shared vision, together with trust and empathy for those we care for. We enable individuals to regain control and independence where possible and we ensure that not only the individual, but their families and friends, are supported through challenging times.

Referrals are made via health professionals or hospitals, direct to the General Manager. A clinical assessment will be offered within two working days to establish if the potential resident's needs can be effectively met on the Neuro Rehab.
